What do I do?

Part of my time is spent on Templar Ward getting to know you to help identify how the Voluntary Sector can support you both in hospital and when you return home.

There is a list of Care/Nursing Homes/Care Agencies/Home Help Services/ Meal delivery service/Advocacy Support/ Meal delivery Service/ Social Groups/ Nailcutting Service/ Take A Break Voucher Scheme/ Hospital Discharge Support from other Organisations.

Having discussed your needs, if appropriate we may arrange:
• To have your heating turned on, refresh your bed if needed, arrange small items of shopping on discharge e.g.: milk, bread.
• A Befriender to visit once a week to spend time with you.
• A Volunteer to help you regain independence and confidence.
• Encourage you to take part in social activities.
• Give you information on local events, activities, clubs, lunches, coffee mornings, Carer support, tai chi sessions & many more.
• Welfare Benefits assessment to check entitlement to Benefits
• Transport to Dr/Hospital appointments usually via Newton Abbot Community Transport
• Signpost/refer to other Organisations who can support you as well.
• Support your Relative, Carer, Friend as well.
• A contact for you following discharge.
